Alberta Asthma Action Plan

In partnership with CANA, the Asthma Centre led a multidisciplinary task force with the goal to create a practical, standardized action plan that was consistent with the national asthma guidelines, and included evidence-based components.  The task force defined the action plan as:

 . . . a tool to facilitate a process which results in written pharmacologic and non-pharmacologic instructions that are guided by action points, developed collaboratively between the patient (and family for pediatric population) and health professionals. It is used by the patient to maintain optimal asthma control and to respond appropriately to loss of control indicators.

Using the Quebec Asthma Education Network action plan as a template, the revised and tested action plan has been evaluated with rural and Primary Care Network teams.
